What is HOPE?
HOPE is a South West District Singapore 21 (S21)
programme aimed at fostering community bonding through:
- Bringing personalised home therapy sessions to the homes
of our needy residents who require treatment
- Engaging befrienders from Voluntary Welfare
Organisations (VWOs) to befriend and take care of these patients on a community
self-help concept
Usually, discharged patients who require follow-up
therapy treatment are either referred to Community Hospitals, or scheduled for
therapy sessions at the hospitals.
At times, prompt treatment is impossible due to the lack
of beds in the Community Hospitals or the inconvenience and cost of commuting to
and from the serving hospitals for the discharged patients.
Upon seeking advises from our partner hospitals on the
local needs of our community, this programme was introduced to supply
personalised home therapy sessions at needy residents' homes.

Who qualifies?
Applicants must be residents of the South West District
with:
- a per capita income of less than or equal to $500 or,
- household income of less than or equal to $2,000
(depending on which is lower), and:
- have difficulties commuting to and from the hospital
- are unable to find placement in the community hospitals
- are awaiting placement in community hospitals but are
discharged to home
How much do I have to pay?
HOPE is heavily subsidised by the CDC. Our residents only
have to pay S$25.00 at NUH or S$10.00 at SGH and AH for each
session. The CDC only subsidises up to a maximum of 3 sessions at 45
minutes to 1 hour each.
How do I apply?
You can inquire and apply with physiotherapists,
therapists or relevant officers from any of our partner hospitals below about
HOPE before you are discharged or outpatient.
